My 2 1/2 year old has her fathers name. He was deported 2 years ago and has provided NO child support. How can I relinquish his rights and give her my name? My daughter's father was deported to Brazil two years ago next month. Since he has been gone he has offered absolutely no assistance with child support. He does not call her, write her letters or even gifts. It is as though she is out of sight, out of mind.

Hi, GAMOM_2012, and welcome to WebAnswers.
First of all, the United States does not have any treaties for the enforcement of child support. Oddly enough, many states have their own arrangements with foreign governments, Georgia apparently not one of them.
In any event, you can file for a termination of parental rights based upon abandonment and, at the same time, petition for a name change. He will have to be properly noticed, although it is unlikely that he will be allowed to appear even if he wanted to do so.

I could not find a termination of parental rights form, but you might VISIT the Cobb County Juvenile Court for a form. I would NOT call them.
